Neurotransmitter Receptor and Transporter Binding Profile of Antidepressants and Their Metabolites
Michael J. Owens, W. Neal Morgan, Susan J. Plott and Charles B. Nemeroff

(Tatsumi et al. 1997, Wong et al. 2000)

The anorectic action of selective 5-HT reuptake inhibitors such as FLU has been extensively studied. These drugs suppress appetite in humans and elicit hypophagia in rodents. FLU produces a suppression of eating by reportedly increasing extracellular levels of endogenous 5-HT in forebrain nerve terminal regions via the blockade of the 5-HT transporter (Romero and Artigas 1997; Trillat et al. 1998). However, the precise mechanism through which this compound alters food intake remains unclear, since chronic 5-HT depletion following raphe injection of 5,7-dihydroxytryptamine does not consistently impair FLU-induced hypophagia (Currie et al.1998).

In the present study, FLU suppressed eating comparably in both male and female rodents. Although there has been speculation that FLU is more potent in reducing food intake in male rats (Heisler et al. 1999), direct tests of this hypothesis, including our own data, fail to show any sex differences. In agreement with our findings, administration of another 5-HT reuptake inhibitor, fenfluramine, is also reported not to produce sex-dependent effects on energy balance (Dagnault et al. 1993). However, we did observe a sex difference with respect to 8-OH-DPAT pretreatment. While 8-OH-DPAT administration alone was without an intrinsic effect on eating, direct injection into either the dorsal or median raphe antagonized FLUs effect in male rats but was largely ineffective in females. Our findings in male rats are consistent with previous work in which systemically administered 8-OH-DPAT attenuated the anorectic effect of FLU (Currie et al. 1998) and fenfluramine (McMahon et al. 1996; Currie et al. 1998) or the suppressant effect of fenfluramine in sham feeding rats (Neill and Cooper 1988). Given that FLU enhances 5-HT neurotransmission, the 8-OH-DPAT reversal of FLU hypophagia would be expected to result from a lowered level of basal serotonergic tone induced by 5-HT1A stimulation.

Other more recent evidence has demonstrated that peripherally administered 5-HT1A antagonists such as WAY 100635 enhance FLU anorexia (Trillat et al. 1998), suggesting that 5-HT1A somatodendritic autoreceptors modulate the effect of FLU through a regulation of synaptic 5-HT levels. Again, the reversal of FLU and fenfluramine anorexia by 8-OH-DPAT appears to be dependent on the integrity of brain 5-HT systems, since their disruption compromises the ability of 8-OH-DPAT to attenuate the feeding suppressive action of either reuptake inhibitor. While our findings extend current work to focus on direct raphe microinjections, they also clearly point to the presence of sex differences in response to 8-OH-DPAT pretreatment.

Unfortunately the majority of studies examining the impact of serotonergic drugs on ingestive behavior have focused in large part on food intake and body weight in male rats, despite evidence of potential sex differences in 5-HT synthesis, receptor binding, and behavioral responsivity. For example, several reports suggest that there are sex differences in peripheral and central 5-HT metabolism. Female rats have a higher activity of 5-HT synthesizing enzymes, a greater storage capacity for 5-HT in central 5-HT neurons, a more pronounced 5-HT behavioral syndrome in response to 5-HT agonists, and higher levels of central tryptophan (TRP), 5-HT, and 5-hydroxy indole acetic acid (5-HIAA) than in males (Rosecrans 1970; Hardin 1973; Fischette et al. 1983; Carlsson et al. 1985). Other data show that female rats exhibit a larger prolactin response to 5-HT1A receptor agonists than male rats (Haleem et al. 1989). Moreover, it is well documented that 5-HT is involved in sexual behavior and sex-related brain functions (Nakano et al. 1992; Jackson and Uphouse 1996; Hillegaart and Ahlenius 1998; Hillegaart et al. 2000; Kelder and Ross 2001). Also, extracellular concentrations of this monoamine vary as a function of the stage of the estrous cycle (Maswood et al. 1999). [3H]5-HT binding is highest in estrus and diestrus relative to proestrus (Uphouse et al. 1986), and estrogen is reported to increase the reuptake of 5-HT at the nerve terminal (Rehavi et al. 1987).

As described above, several studies have indicated that female rats are less sensitive to the hyperphagic effects of 8-OH-DPAT (Uphouse et al. 1991; Ebenezer and Tite 1994) and it now appears that estrous cycle modulation may play an important role in this diminished responsiveness. Sex hormones influence central 5-HT receptor gene expression, binding capacity and function (Fischette et al. 1983; Zhang et al. 1999). Uphouse et al. (1991) have reported that 8-OH-DPAT is ineffective in stimulating eating in proestrous and estrous rats, when endogenous levels of estrogen are high, but does elicit hyperphagia in diestrous rats, characteristic of low estrogen levels. Moreover, Salamanca and Uphouse (1992) have demonstrated that the hyperphagic effect of 8-OH-DPAT is attenuated if ovariectomized rats are administered estrogen. Although we did not observe eating in either male or female rats in response to 8-OH-DPAT treatment because we deliberately administered subthreshold doses of the agonist, it is likely that female gonadal hormonal influences contributed to the inability of 8-OH-DPAT to reverse FLU anorexia of female rats in our study. This is supported by our additional finding that 8-OH-DPAT was effective in altering the anorectic action of FLU in ovariectomized females but not sham-operated controls.

Consequently, estrous cycle modulation of the 5-HT1A receptor subtype might be expected to have a significant neurophysiological impact by influencing the firing rate of raphe neurons and ultimately the synthesis and release of forebrain 5-HT. While earlier work demonstrated that estrogen inhibits the ability of 8-OH-DPAT to reduce the firing of dorsal raphe neurons (Lakoski 1988), it is now evident that both estrogen and progesterone may contribute to the estrous cycle modulation of the 5-HT1A autoreceptor (Maswood et al. 1995). Of course, the relative contribution of male gonadal hormones must also be investigated.

In previous reports, systemic administration of 8-OH-DPAT would be expected to activate somatodendritic autoreceptors in addition to 5-HT1A receptors on neurons in the terminal fields of the forebrain. Our findings focus on an action exclusive to the presynaptic 5-HT1A autoreceptor. They are also in agreement with the view that female rats may exhibit less autoinhibition at 5-HT raphe neurons and, as a result, might be predicted to exhibit less evidence of a behavior requiring 5-HT1A autoreceptor activation (Uphouse et al. 1991). This line of reasoning is consistent with female rats showing less sensitivity to the hyperphagic action of 8-OH-DPAT and a reduced sensitivity with respect to the drugs ability to reverse FLU anorexia. Collectively, these findings suggest that gonadal hormones influence brain serotonin circuitry and, in particular, the differential responsiveness of female rats to 5-HT1A somatodendritic autoreceptor stimulation.

However, the precise mechanism through which this compound alters food intake remains unclear, since chronic 5-HT depletion following raphe injection of 5,7-dihydroxytryptamine does not consistently impair FLU-induced hypophagia (Currie et al.1998).
